how do you properly measure groups??

from outside edge of hole or outside edge of mark that the lead left on the paper?

You want to use the outside edge of the true bullet diameter. If that means the lead mark then use that. Otherwise use a scoring gauge or overlay to determine the dia of the bullet hole in the target.
